Testimonial Release Agreement 
For proper consideration, receipt whereof is hereby acknowledged, I hereby agree as follows:
I represent that I am over 18 years of age and have the ability to enter into this agreement.
The testimonial I am giving to Scandinavian Clinical Nutrition, LLC. “SCN” is an expression of my personal experience and belief.
My statements, the photographs of me and the recordings of my voice used and/or taken by or on behalf SCN shall be owned by SCN and SCN may copyright any material containing same.
I hereby grant to SCN and its successors or assigns the right to publish and use any statements made by me, my voice and likeness in any marketing materials created by SCN that are used in connection with the promotion of SCN and/or its products.
I agree that no marketing materials need be submitted to me for any further approval and SCN shall be without liability to me resulting from the publication of my name, likeness or testimonial.
I warrant and represent that my agreement with SCN does not conflict with any existing commitment or agreement on my part.
I acknowledge that nothing in this release constitutes any obligation by SCN to use my statement name, likeness or testimonial.
This Agreement shall be construed and enforced pursuant to the laws of the State of Illinois. If any term or provision of this agreement is found to be invalid, the validity of the remaining terms, clauses and provisions shall not be affected. If any challenge or dispute pertaining to this agreement arises, the prevailing party shall be entitled to recover all costs and expenses, including reasonable attorney’s fees. This Agreement contains the entire understanding and agreement between the parties hereto with respect to matters referred to herein.
No other representations, or prior or contemporaneous agreements, oral or written, regarding such matters which are not specifically contained herein shall be deemed, in any way to exist or bind any of the parties hereto, except as specifically set forth herein.
All materials supplied by you shall become the exclusive property of SCN and shall not be returned.
